Visitors Waiver

G13 BRAZILIAN JIU-JITSU, LLC
RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT
This Release of Liability and Assumption of Risk Agreement (“Agreement”) is entered into by the undersigned participant (“Participant”) in favor of G13 Brazilian Jiu-Jitsu, LLC (“G13BJJ”).
In consideration for being permitted to participate in Brazilian Jiu-Jitsu, Muay Thai, martial arts instruction, fitness training, open mat sessions, and all related activities (collectively, the “Training Activities”), Participant agrees as follows:
1. Acknowledgment of Inherent Risks
Participant understands and acknowledges that martial arts and physical training activities involve inherent risks, including but not limited to:
physical contact and grappling,
falls and impacts,
sprains, fractures, or other serious injuries,
permanent disability, paralysis, or death.
Participant voluntarily chooses to participate with full knowledge of these risks.
2. Assumption of Risk
Participant knowingly and voluntarily assumes all risks, both known and unknown, arising from participation in the Training Activities, including risks resulting from the ordinary negligence of G13BJJ or other participants.
3. Release of Liability
To the fullest extent permitted by North Carolina law, Participant releases, waives, and discharges G13 Brazilian Jiu-Jitsu, LLC, its owners, officers, instructors, employees, independent contractors, agents, and representatives (“Released Parties”) from any and all claims, demands, damages, or causes of action arising out of or related to participation in the Training Activities, including claims based on ordinary negligence.
4. Covenant Not to Sue
Participant agrees not to file or pursue any lawsuit or legal claim against the Released Parties for injuries or damages arising from participation in the Training Activities.
5. Medical Authorization
Participant certifies that they are physically able to participate in Training Activities and have no medical condition that would increase risk of injury.
In the event of an emergency, Participant authorizes G13BJJ staff to obtain emergency medical treatment if deemed necessary. Participant understands that all medical expenses are their sole responsibility.
6. Minor Participants
If Participant is under 18 years of age, the undersigned parent or legal guardian:
consents to the minor’s participation,
acknowledges and assumes all risks on behalf of the minor,
agrees to all terms of this Agreement.
7. Governing Law
This Agreement shall be governed by and interpreted under the laws of the State of North Carolina.
8. Severability
If any provision of this Agreement is held invalid or unenforceable, the remaining provisions shall remain in full force and effect.
9. Acknowledgment of Understanding
Participant acknowledges that:
they have read this Agreement carefully,
they understand its contents,
they understand they are waiving certain legal rights,
they sign voluntarily.
